Management of Anticoagulation for Patients Undergoing Procedures



The WSLHD Perioperative Management of Anticoagulation for Adult Patients Undergoing Surgical or Medical Procedures Guideline (version 1.0) has been recently approved and published. It is now available via the Policy and Procedures Portal.

This document provides guidance for anticoagulation for adult patients (over 16 years of
age) undergoing surgery or medical procedures in Western Sydney Local Health District

It is based upon the NSW Clinical Excellence Commission (CEC) Guidelines on Perioperative Management of Anticoagulant and Antiplatelet Agents (updated December 2018).

Change of Intravenous Heparin APTT Therapeutic Ranges from 4 May 2020



From 11.00 am on 4 May, the APTT will be measured with new reagents and therefore the APTT therapeutic/target ranges for monitoring therapy with intravenous (IV) unfractionated heparin (UFH) will change.

As a result, the WSLHD Anticoagulation (Adult) Guidelines version 12.0 have been revised to reflect the new APTT therapeutic ranges. Please refer to the published electronic guidelines on the Policy and Procedures Portal for the latest version.

The reference texts in the eMeds Heparin Powerplans have also been updated to reflect the new APTT therapeutic range for IV unfractionated heparin.

Telehealth with PEXIP



PEXIP is the Telehealth platform for NSW Health. Pexip is a web-based telehealth tool that allows clinicians a secure platform to remotely connect with patients and families for telehealth appointments. Instead of attending a face-to-face healthcare appointment, technology is used to connect everyone involved from multiple locations, including private locations (home, work) or other health facilities.

You can apply for a PEXIP account by logging a request via SARA. Instructions on setup and training materials will be sent to your Health email.

PEXIP training is now available for NSW Health staff on the My Health Learning website.

The online, instructor-led training sessions will cover:

  • connecting via a computer, room-based solution and tablet

  • call control features

  • host control features

  • technical support

  • where to start

  • resources.

Duration: 1 hour

How to enrol: 
Log into My Health Learning and search for ‘Pexip’.
Click ‘Enrol’ on your preferred session.
